Incidence of Neurogenic Shock

چکیده
Neurologically injured patients, regardless of the nature of the injury, frequently experience hypotension and shock. Neurogenic shock refers to a neurologically mediated form of circulatory system failure that can occur with acute brain, spinal cord, or even peripheral nerve injuries. In this chapter, we will explain the epidemiology, pathophysiology, clinical presentation, and management strategies for this special form of shock. Contrary to common belief, neurogenic shock is not a single entity due to one single pathologic mechanism. The term is sometimes used in nonneuroscience intensive care units to explain hypotension occurring in any brain-injured patient, but neurogenic shock should be considered only after systemic causes of shock have been carefully ruled out. Just like other critically ill patients, neurologically ill patients are prone to developing systemic conditions, such as dehydration, congestive heart failure, acute blood loss, sepsis, pericardial tamponade, or massive pulmonary embolism.

The incidence of neurogenic shock in patients with isolated spinal cord injury in the emergency department.
BACKGROUND Spinal cord injury (SCI) is recognised to cause hypotension and bradycardia (neurogenic shock). Previous studies have shown that the incidence of this in the emergency department (ED) may be low. However these studies are relatively small and have included a mix of blunt and penetrating injuries with measurements taken over different time frames. The aim was to use a large database t...

BACKGROUND Neurogenic pulmonary edema (NPE) is a clinical syndrome usually defined as an acute pulmonary edema occurring shortly after a central neurologic insult. NPE was identified 100 years ago, but it is still underappreciated in the clinical setup. NPE usually appears within minutes to hours after the injury. It has a high mortality rate if not recognized early and treated appropriately. S...
متن کاملNeurogenic Shock
Neurogenic shock is a type of distributive shock that describes the sudden loss of autonomic tone due to spinal cord injury often characterized by hypotension and relative bradycardia. Loss of sympathetic tone occurs with injuries above T6 and results in decreased systemic vascular resistance. Peripheral vasoconstrictors, chronotropes, and inotropes may be needed in cases of neurogenic shock. A...

1 The dopamine-induced neurogenic vasodilation, previously described in the isolated perfused hindleg of the dog, has been studied in anaesthetized dogs with intact circulation in the hindleg. Dopamine was administered intravenously and/or intra-aortically, either as a bolus injection of 4 or 1l mug/kg, or as a continuous infusion of 4, 8, 16 or 32 mug kg-1 min-1. 2 Dopamine, given as a bolus i...

We report a rare case of a 71-year-old male patient who had suffered from long-lasting neurogenic shock for 13 weeks after cervical spinal cord injury (SCI) caused by a bicycle accident. The neurogenic shock was resolved dramatically 2 weeks after the administration of alpha-1-adrenergic agonist, midodrine hydrochloride. In usual cases, neurogenic shock tends to improve between 2 and 6 weeks af...
